Tier 1 General Dependent application form - few queries!

Hi,

I have a few queries in filling the following fields in the form of dependent application (switching from WP dependent to Tier 1 dependent from within UK). Please help me complete the form:

1. F10 - F13: Previous Fingerprints details:We are here on work permit and we gave finger prints (biometrics) before we came here but obviously i dont remember the details like exact date and the officer who took the fingerprints. What should i fill here? Is it mandatory? Also this details in only required in Dependent form and not Main applicant form. Why so?

Hope to see the responses soon as I have to apply early next week.
