Health care unit (heathrow terminal )

I am asking on behalf of my friend who recently arrived in UK he has been given Health care card by immigration authorities and on it he has to write where he will be residing from ... too.... and phone no .... why is that form for????please does anyone went through same experience ..

everyone has to fill a form at the airport saying where u are gonna stay..

Are you sure it is a health care card? What is the VISA status of your friend? What led the immigration authorities to issue one to him/her? What was the discussion he/she had with them?

After filling the form we have to return it to

The principal medical officer
Health Control unit
terminal 3 Arrivals
Heathrow Airport
Hounslow
....

He is on student visa status and he is overall well.
