Tier1 Dependent for a New Born

Tier1 Dependent for a New Born

Hello All,

I need your help for filling the depandent visa application for my New born baby.I am on Tier1 visa in UK.
I have listed queries below for your kind responses.

1) Documents need to attach ?
i) baby's passport
ii)baby's birth certificate
iii)Dependent Application form
iv)My last 3 months bank statement - minimum amount £533
v) Copy of My passport (front & last page)
vi)Copy of my Tier1 Visa

Please add/correct if I am missing any thing.

2)Depandant application form queries?
Section D5 - Please enter the reference number on the entry clearance or visa which can be found in the dependant’s passport or travel document.
Similarly for D7 to D12 about entry clearence details.
Section D25. Was this application submitted before the dependant’s leave expired?
Section D26. Has the date of the dependant’s leave, as stated in his/her passport, now passed?
My baby born in London so did not have any visa or entry cleaence yet - What should I write/fill for these sections?

3) Section J4 - Has the main applicant applied to have his/her application assessed in accordance with paragraphs 39-42 of the HSMP Forum Ltd. Judicial Review Policy Document?
Where I can find this paragraphs 39-42?
I switched from workpermit to Tier1 in July 2008.

2) Put "Not Applicable - born in the UK (304-309)"

Thanks alot Vinny - but these are like table coloumns so should I leave them blank or fill NA ?

All - Can you please advise on point 1 & 3 ?
