when does spouse visa application apply from?

We got married in august 2009, and hav eonly jsut been informed, that we need to get a spouse visa before his post study work scheme visa runs out in oct 2010.we assumed we had to wait until near the end of his current visa.

so if we apply for spouse visa now, will it begin from the day we married, or on application date

also..we married in pakistan ...he has been here 3 years already we both went there to get married, im a uk citizen, we then came back in sep 2009, so does this come into effect? does it matter we didnt marry here in uk, we were told not to bother getting `married/registerd` here,because we wrre already `married`..so we didnt need a certificate of approval..
