Application form and policy guidance.

D1, D2 - Infant applicant born in the UK.
D3, D4, D5 - Not Applicable.

F15 - Understandably, either the father or the mother. Ideally, the main migrant.

For document list, read the policy guidance and make an effort to search the forum as the information is already available.
