Tier 2 Application form questions

Hi

I am currently on PSW and applying for Tier 2 .I would like to know the answers to the following questions on the Tier 2 application form.

1) B13 Points Based System migrant reference number( What number is this??)

2) D2 Where did the applicant obtain entry clearance or a visa before travelling to the United Kingdom? (Is this referring to my PSW or my student visa???)

2. these are the details of the 1st visa u got to enter UK

Thank you Ramesh but I certaintly read somewhere in these forums that it is referring to my PSW. I could be wrong but just double confirming it.
