Bear in mind that even if the rules are not changed, it might be that you do not meet the requirements (income, etc).
2) No one knows for certain how the cap will be implemented. A cap might be difficult to impose on extensions, but tweaking the rules to reduce the number of extensions (but keeping the best or most needed people) would be fairly easy. That is just my thoughts, there are other threads on here where there is other speculation about what might happen.

I can't imagine it would look good to the public if they start chucking out NHS doctors and nurses so targeting extensions seems unlikely. But who knows.
