Tier1(G) dependent help needed (urgent)

Tier1(G) dependent help needed (urgent)

Hi,

I am in UK for around 5 years and I got my tier1(G) last year and then i applied for my wife's entry clearance from Pakistan which she got it but for some unknown stupid reason her visa hasn't got the same validity as mine, her visa is expiring next month and she is here with me. i am applying for her extension in 3-4 days n got few queries to sort out so please reply

the documents i am submitting are

visa form with fees £475 right?

maintenance funds: i am providing my bank statements to show that I've got more then £1333 in my account for the last 3 month, bank statement has only got my name, is it ok?

marriage certificate in English
few photographs of ours
my employment letter
do i need to provide payslip as well?
my visa copy

PBS(dep) policy guidance's point 105 says that applicant need to provide at least two proof of address, but my wife has only got NHS card and no bank statements or credit card bill, is just NHS card enough ?

if i am missing any document please let me know

Personal bank or building society statements covering the three month period immediately i) before the application. The personal bank or building society statements should clearly show:
• the name of the main applicant and/or the family member making the application, or (for children only) the name of the other parent who is also legally present in the United Kingdom;

hi again,

last week i applied for the extension of my wife's dependent visa and provided the documents mentioned in my earlier post, along with those documents i also provided the certified photocopy of my passport and visa (3 pages only and not every single page).

have i done a blunder? i am so worried now, please reply
