Spouse Visa

Dear All,
I am on Tier 1 Extension which is expiry on March 2013. I have been marriage 1.5 years ago when I had visit to Pakistan. My wife is still in Pakistan and now I am going to apply her visa. I would really appreciate if someone answers the following queries...

1) How can I show that we were in touch during that period?
2) Total fees?
3) Total Time?
4) Any list of documents would be highly appreciated.
