I am new here. This is a wonderful site for everyone applying for Tier 1.
I have been in UK for some time now. In fact my old Tier 2 WP got expired and I got a new one (card format) just a few months back.
I have most of the things done(thanks to friends) except a few queries:
1. Many people in this site are filling some VAF 9 form with appendix 1. But in UKBA site only this form is given for in country applications:

I assume this is the correct form since I have filled this only?
2. On this form I have filled up most except a few queries:
a. B10 and B11: HO reference numbers and Worker ref number are the once given in my WP application. Correct?
b. B12: I never applied for NI number but was given one in tax returns. Can I use that number?
c. Point based system migrant number, B-13: Not applicable to me since this is my first tier 1 application?
d. UK ID card number: Is this the card number for my new WP(plastic format). Is it different from B11.
3. On Page 15, I am a IT consultant. So I should choose Science and Tech professionals. Correct?
4. Page 20: D3: Date of entry clearence or visa issued
D4: Refnumber on entry clearence or visa
Lot of confusion on these two? I travelled to home for few days in vacation. Should I fill those dates?
These may look trivial but since I am applying first time and most of my friends are confused on these. Just dont want to take any chance.
Also one of my colleague is applying with Indian salary slips.He has stamped and signed e-statement from his bank but as per guidance also needs a letter on bank letterhead. Can someone give format for the same. It was not available on this forum.
