Fresh (initial) or Extension application: Please advise

Friends,

My spouse works as a Junior doctor (Foundation year 1 doctor) on Tier 2 General which expires early August.

Now she has got another job (Foundation Year 2 doctor) in a different hospital, but under the same sponsor (same deanery).

Now my query is, can she make as an extension application of original Tier 2 or does it have to be a new initial application (job title slightly different, different location, but same sponsor)

as she has changed jobs; her sponsor must apply for a change of employment application....before her visa runs out.
