IELTS details in the new APPENDIX 8

I have an IELTS certificate and I'm confused what to answer the following questions:
5.11:
Awarding Body:
Title of qualifcation:
Level of qualifcation:
Award reference number:

British Council(If you have given exams in a Cambridge centre then write ESOL Cambridge.
Title of quantification:
International English Language Testing System(IELTS)

ESOL LEVEL 2(Note:this is my search..better double check that)
Award reference number:
I am bit confused here. There are two reference no on Test Certificate.

1.Candidate No

2.Test Report Form No

I belive second is a good option.But I am not sure on that

This is my opion only so please double check that.
