Tier 2 General (Me) & Tier 1 PSW (dependant)

Hi Experts & members,

Will appreciate the help with the followings :--

I am in UK in Tier-2 General since Feb 09 and living in UK more than 2 years and my wife ( married in UK by COA for marriage ) in Tier -1 PSW.

Q.1 Can my wife able to apply in UK to extend her visa as dependant of tier 2 general.
if yes
Q.2 How many PBS (DEPENDANT) application form we need to fill up?
Q.3 Who need to show the £533 maintenance? me or my wife ?
Q.4 Do I need to show my Maintenance of £800 again or £533 for my wife only?
Q.5 Do we need to include my Passport & Biometric Residence Permit with the application or My Tier-2 reference on the application from will be enough?
Q.6 Do we need to present the Marriage certificate?

Well lots of Q's but I cant find any clear answer neither on home office guidance nor in application form .
