dependent visa

I am on old work permit issued on 2007. my baby born here in uk. but we didnt apply his dependent visa here in uk.

They went back to home country . now i want to bring my baby to UK alone with my wife . i know my wife already got her dependent visa.

would you please tell me what visa form my baby need to fill up for dependent visa.

** i am on old work permit NOT on point base system.
