Tier 1 dependent - 4 months baby

Hi,

I would like to apply dependent Visa for my wife and 4 months old baby. Do I need to submit separate Vaf10 for my baby and will thumb expression be required in place of signature?

Also will these documents be sufficient for dependents -

Marriage Certificate along with marriage card and photographs
Birth Certificate (baby)
Passports
My passport and Visa copy
Bank Statement for last 90 days for fund maintenance
Sponsorship and cover letters (please send me formats for these)

You need to fill separate forms for your dependants.
