need form for tier-1 general (applying from outside of uk)

hi,

i have gone thru all the instructions and guidelines for tier 1 general visa and found out that i am eligible for the same. i m applying from india.

can someone tell me how should i proceed with the visa application on my own. i need to know which form to be filled in and where can i get the same.
is there only one form to be filled or there other forms and annexure as well.

You have fill 2 forms,

1. POINTS BASED SYSTEM FORM (VAF9 MAR 2009)
2. POINTS BASED SYSTEM APPENDIX 1(JULY 2010)
