Child born in UK for parent holding TIER1 Visa

Hi,

I am holding a TIER1 Visa valid till 2012 April.
My child was born in the UK 3 Months ago and I have secured an Indian passport for her.
Which form should i use to apply for her dependent visa.
The UKBA website is only providing links to "extension of stay as a dependent in the UK" .
Should i be using this application?
Or alternatively could you share the link for application form.-dependent visa for children born in the UK and the application is made within UK

Will the child be traveling outside the UK?

Yes. the child will have to go to India and return

I was in same situation last year and i used "extension to stay as a dependent in the UK" and it worked alright.
