Registration of UK Born child as a British National

I will soon be applying for registration of my child as a British National. I like some information before i proceed for submission

1- Is MN1 form to be used for registration
Please note that both the parents have been granted ILR under the terms of HSMP Judicial review and Child was born in UK before we got ILR.

2- I need to know what doc are to be submitted with the MN1.
I have got a copy of my marriage certificate but the original Translated & attested marriage certificate has never been returned to us by British High Commission when we applied for Entry clearance.

Also like to know about the payment as there is no payment section on MN1 form.
