Does NADRA Pakistan do B-Form in English?

Hi Experts,
I am going to apply for my dependents soon. I need birth certificate translated in English. I wonder, Does NADRA Pakistan do B-Form in English? Or I have to have B-Form (which is currently in URDU) translated to English from some Translator.

One more thing, Do My dependents need my Passport Visa Copies Attested by notery public service or just copy is fine?

Hi Salloo,

B-Form is now called Family Registration form which is issued by NADRA in both Urdu and English. Basically the same form will have all the details written on it in both the languages.
