Tier1 Extension with dependent

Hello Friends,
I cant find much threads for Tier1 extension as its just few people have started only.
I'm planning to extend Tier1 with my wife who is already in the UK, but went through the docs and found these.
1. Need to complete full PBS form for extension + a dependent form ( i meant to say its not in single form)
2. Planning to apply via post, but still need to have biometrics.
3. Planning to show only 10 months of Salary, (the required points only)

a. what is the proof I need to show for the dependent application? Should i show the marriage certificate, photos etc again or just joint council tax statement?

b. Is there any separate form for Tier1 dependent extension? Hope thisis the document

I showed the council tax statement and the marriage certificate. Although just the council tax statement should do; I guess it is better to show both just to be safe.
