Hi, Please guide which applications to fill?

Myself have got the ILR August 2010 as under long residence before that I were in student category. My wife and I had got the student extension (wife's under dependant) in 2007 for 4 years which is expiring on April 2011. OK! Good news are I got my ILR and a baby born in last year March.

Question? Which Form set my have to fill for an extension? Do I HAVE to include by baby in to mother's visa? (she' has born in UK) please advice?
