Lost Passport with spouse visa HELP

Hi,

Please advice, a friend of my lost his Passport with his UK Spouse Visa last Month.

He Applied for new passport at his Embassy, and got it Today.

now there are 3 Questions:


1. What form is needed to re-apply for the Visa
2. Can he wait and apply for his ILR as he entered UK 09/2009
3. How Much is the replacement cost for a Spouse VISA.


PLEASE PLEASE HELP OR ADVICE.

Any Advice would be great.
