Ok so you have now found your answer to 2)

Try the PSW policy guidance for 1) and 3).

Try the UKvisas website 'guide to visa processing times for 3).

It's really as easy as that.

the form is here

you also need to complete the relevent appendix, which you can find on the same page.

1. You should maintain 2800 pounds for 90 consecutive days
2. You can download the application from here the direct link is
http://www.ukvisas.gov.uk/resources/en/ ... 03073/vaf9

and you need the appendix which is at
http://www.ukvisas.gov.uk/resources/en/ ... tstudywork

3. Documents you need
i) You should include a letter from your University where you studied, its called a PSW Letter and your uni will know all the details to put in.
ii) your bank statements covering 90 days with balance not falling below 2800 GBP or equivalent to 2800 in foreign currency.
ii) Your degree certificate if you have received it, otherwise PSW letter is enough. If you include your transcripts it is better.

4. Depends on the embassy in which you apply as for Hyderabad 39% applications are processed in 5 days, 87% in 10 days, 93% in 15 and 99% in 30days
