The new version of the EEA4 application form covers now most circumstances, also correct me if i'm wrong according to this new form if your EEA National has acquired his/her permanent residence documentation you can now use it solely as an evidence to prove they have been exercise their treaty rights during your five years of residency rather than sending all their p60s,contracts,wage slips and letters from employers.It's a really improved and straightforward version
