Tier1 G Extension query on immigration history

Hello,

I am about to apply for Tier1 general extension. I have a query around immigration history in the form.

I got my wp on 02/2006, came to UK on 03/2006.

Was in the UK till 05/2007 and after a 6 month gap came back on 11/2007 with the same WP. Then applied for WP extension through my employer. Meanwhile I applied for Tier1 general on 2008 and got it approved on 08/2008 - valid till 09/2011.

My query is on the following questions:

D2. Where did the applicant obtain entry clearance or a visa before travelling to the United Kingdom?

What date was the entry clearance or visa issued? dd/mm/yyyy

D4. Please enter the reference number on the entry clearance or visa which can be found in the applicant’s passport or travel document.

For all of the above questions I have provided the details of the WP obtained on 02/2006. Hope that is right. Please clarify.
