I just know that EU citizens MUST have privat inssurance and not use NHS to be able to apply for BC. No info about NON EU. but for a matter like this you should speack to Home Office or Immigration Lawyer.

Did you wife apply for a Residence Certificate as a EEA national as a student? Because although CSI is required, the HO is waiving this requirement if you have applied for Residence Certificate (EEA1 application) as a student before June 2011. If this is the case, I would either try another NCS or postal application. If you prefer not to take the risk and willing to wait, then just wait until August 2012 and you will not need to provide your wife history when applying.

In that case, I would try another NCS as it is quite useful to use their service and keep your documents or you could apply by post.

If the HO waived the requirement for CSI, there is no reason to claim you have not obtained PR automatically after 5 years and should allow to apply for BC after 5 year + 1 year.

You will also need to pass the life in the UK test.
