An information how to transfer a RC document in to passport

Can anyone in this forum enlighten me on how to transfer a RC document in to my new passport, the HO number to contact, the duration and the cost of fees for processing & am strongly sure there is no need for me to make a new application in other to get it transfer, thanks for your time
