Irish and UK defacto relationship application

Can somebody help us please....

Me and my unmarried Irish partner were living together for 5 years in Ireland. We applied the de-facto relationship 2 yrs ago and were granted stamp 4 status. Now we are looking to move to London to look for jobs thus need to reapply the UK version of de-facto. I have been looking at ukba.homeoffice website to find the suitable form for our application but jaysus, they are confusing....Can anyone tell me which is the right form? EEA2, VaF5 or other form....
All advice are welcome. Thanks
