please, I can not remember my dates in the previous addresses. application based on 5 years and do I need some evidence for the past 5 years?
got all the residence visa in my passport.

IF you can't remember the exact date, just put the month + year. That's fine. There is no need to provide evidence for the past addresses.

If your passport is stamped on every entry to the UK, then a passport is all you need to provide as evidence for residence in the UK.
