This external website outlines the application procedure (which apparently UKBA has shared with IPLA, but not made public as yet).

In the absence of any form, write a covering letter stating that you are applying on the basis of the zambrano and include evidence of the relationship btw u and the british citizen like birth certificate if you are a father of a british citizen, show evidence of living in same address - yr bill and school letter or GP's, show evidence of financial relatioship too

i spoke to one of the ukba's staff a couple of minutes ago and she knew nothing about the zambrano judgement. i told the lady that this information was released on 21/09/2011 and its there on their website, she searched and found what i was talking about. she put me on hold and went on to inquire as she have not received any training on this judgement. she said anyone affected by this judgement needs to use an EEA2 form as they is no form yet. she said the application can take up to six months.
