Tier 1 extension form - Other Name

Hi Everyone,
While I was filling the Tier 1 Extension Application form, I have doubt on
B4 Section : If the application ever known by any other name.

Before my passport my name was Punit which I got changed in India thru affidavit to Puneet, my passport has the changed name (Puneet) but my Education certificates have Punit name. I don't have that affidavit now?
My questions are :- 1) Shall I mention about the previous name?
I have sent the affidavit during my initial application, 2) Can I mention Name Change Affidavit already given during initial application.

please suggest.

No harm in sending the affidavit again with this application as well. You could mention the same in cover letter
