How to fill application -PSW form

Hi ,

This might be simple question but it's tricky , can any one please suggest how to write in boxes for example after first name should we leave a blank box and then write last name .Secondly for address suppose we have to write 40,ABCD street how we will write ? do e write 40 space obe block , space other block and start with street name.Would be thankful for help.

Just leave a blank box where you would normally make a space
