Need help filling MN1 form?

Me and my wife today 22-11-2011 got ILR after completing 5 years on HSMP. My son born on 21-08-2011 in UK and I did not include him in ILR application.
So I am going to apply MN1 form for my son. I am having little difficulty in filling MN1 form.

Q:1 what date I have to mention in page 3, where it says child has granted ILR?

Q2: In section 1.5 on page 3, Surname/ Family name. Do I write full name or just surname of my son? coz I dont see section for first name

Q3: section 1.9 page 3, what I write in Present Nationality of child?

Q3 same as yours and your wife nationality, if they the same.

1.5 asks for just the surname so just write the surname.
1.6 is for other names including first name.
