HSMP Extension-application form + documents...HELP

Hi everyone,
I am new to this forum! You guys are amazing, I have learnt so much on this board, thank you!

The thing is, I plan to apply for my HSMP extension in October/November (expire on 7/12/06). I have read some articles and downloaded the application form. I have some questions so I e-mailed HO last week, today they replied me..HOWEVER, due to huge amount of application, they don't have time to answer my question. Instead, they forward me some 'FAQ'...which is almost useless:(

So, my question is:
1.passport
I came to London in 2004 with my old passport, when I applied for HSMP last year I used my NEW passport (and attached the old one), so the work permit is on the new one.
Do I need to mail both my old and new passport this time? or just new one?

2.Section 3, Question 19
'From which British Diplomatic post overseas did you obtain entry clearance and/or visa?'
When I came as a student, I went to the representative office in my country. I apply my HSMP here in London and I don't leave this city ever since. So how am I gonna answer thsi question?

3. Do I need to enclose my house rental contract?

4.Economic Activity
I got my job in Jan (1 months after I got my HSMP), I plan to show :
-Bank statement Dec/05 - Feb/06
-Wage slips Feb/06 - Oct/06

is it ok? or should I send all of my bank statement along with my wage slip?

Name the post where you got the student visa issued.

Yes

Bank statements & wageslips for the last 4-6 months will suffice. Include your employment contract as well.
