For my new born - HSMP Forum Ltd. Judical Review & more

Hi

I am under Tier 1 (General) visa which I received in Oct 2009.
I am now filling up the PBS (Dependant) Version 07/2011 for my 11 months child.

I came across the below question but have absolutely no clue as what to answer:

J6. Has the main applicant's application been assessed in accordance with paragraph 39-42 of the HSMP Forum Ltd. Judical Review Document?

Please can some one help?

Also if I could also ask:
They have asked for a home office reference and PBS migrant reference numbers if I have got any.
I believe I should have them but please can anyone guide me on where to look for them?

As you hold Tier 1, you have to answer NO to the HSMP judicial review question.

Home office and PBS migrant reference are not mandatory as it says (if known) in the application form.
