SAR Application Form

Hi,

Do we have any application form for SAR or just letter on a plain paper with proof of identity like driving licence.

One thing more..If I have to apply for SAR of my dependent, does she need to apply separately?

Please you don't application form for SAR.
On your letter state that you want complete data and they include your dependants.

I would be grateful if my complete personal information i.e. all documents which the Home Office holds about me on their system is sent to me to the postal address as stated above.

In case you need any further information from me, please do write to the address stated above
