Which form to use for extension???

Hi
I applied for ILR last week but unfortunately I was rejected because I had a 4 months break in my stay. Today I got my passport back with a letter saying that I need to apply for an extension now in order to qualify for ILR in May 2012.
Right now i m tier 1 general
I m not sure which form Should I use for my extension application.
can someone give me a link please .
AM I supposed to use this form http://www.ukba.homeoffice.gov.uk/sitec ... idance.pdf

or Further Leave to Remain Tier 1 (HSMP) 04/2011 because I also got judicial letter from before I got my first HSMP.
initially I was HSMP migrant.

thanks in advance to anyone who can tell me

When was your original HSMP visa issued (the visa itself, not the approval letter)?
