Tier 1 extension english language test

Hi,

Per this it mentions that it is mandatory to take an english language test for Tier 1 extension or for filing a fresh dependant visa?
Couple of queries:
1] I currently want to file for an extension, does that mean I will have to give a test even if I had previously proved my english language proficiency (Degree done in english)
2] If I need to get a dependant then does the same rule apply i.e. does she need to take an english language test or the dependant can show her degree in english?
