I see it marginally differently and to answer your question at the end of this assumed salary table, one needs to understand that when you are paid on 15th Jan 2011, what period is that salary for. I use to work for a company. I joined end of March 2011. Company policy for salary was 15th of the month. So my salary from 21/03/2011 - 30/04/2011 was paid on 15th April 2011. HOWEVER, when I will appear for my extension (in March), I will count my 12 month period from March 2011 - February 2012.
Hope this makes sense. Now coming back to your table - If you are paid on 15th OF each month, does your salary period is 15th - 14th of the month? If so, your assumption below is correct (i.e your 12 month will be 15/1/2011-14/12/2011) However, if your salary period (for the salary paid on 15/1/11) was 1st Jan - 31st Jan, then you should consider your 12 month as 1/1/11 - 31/12/2011.
