HSMP+Tier1+ilr

Hi All I Got my HSMP in March 2007 (I was already in the UK on student visa) then got extension under tier 1general in March 2009 and now Due for ILR this month and will apply same day service, I will take the following documents with me please let me know if something is missing.
Regards

1) Set(O) form
2) Passport
3)2 Photos
4) LIUK test
4)12 months bank statements
5)As I am a self employed with a company so Invoices for the last 8 months (as I just need 24K to score req points)
6) Accountant's letter
7) last 5 years of P60s and HMRC tax returns (for self employments)
8) No Offences, total absent is 78 days in 5 years.

Please Advise me if I need anyother document as well.

If you are claiming to be "self employed", do you have any registration with HMRC with UTR as Self Employed?
Do you pay any Class 2 National Insurance?

Hi thanks Yes I do have HMRC letters for NI contribution and Bank statement transactions as well.
