Tier 1 (General) Extension from UK

I am main TIER1 (General) applicant. 1st applied 30Dec2008, Got Visa in Feb 2009 valid from 1st March 2009 for 3 years. Landed 14 march 2009. I came with my wife&son (dependents). 1st march 2012 Visa expiring for all of us. Now want to apply for 2 years extension. I have found TIER1 form online version 04/11. Is this the correct form/version?

Do I have to fill a separate form for my dependents or just fill the above version form, provide related proofs and send all 3 passports together ?

Any checklist link to attach all related documents/proofs?
Running a Limited Company.

How much is the Fee? Main Applicant? two dependents?
