if your wife's initial visa or any other UK visa was on her old passport then fill in this section, with an answer to the above question as 'Yes'

I think HO is asking for ANY PASSPORT (which has Tier 1 Inital Approval stamp)
H1. Have you ever had any Home Office or UK Border Agency reference number(s) assigned to you?
I think , when I got Tier 1 Inital visa approved I had Visa stamp + Letter HAD reference number on top right hand side
L3. Reference number of the main applicant’s most recent application to us:
SAME AS
I think , when I got Tier 1 Inital visa approved I had Visa stamp + Letter HAD reference number on top right hand side
L4. Date of the main applicant’s most recent application to us:
I think HO is asking for APPLICATION Date not VISA approved DATE

As Greenie said, the answers provided by pgaitonde are what you just need as they are all correct. I think you got the answer to this question slightly wrong. As pgaitonde said, this needs to be
your wife's initial visa or any other UK visa in the old passport not just tier 1

It will be YES. In any condition it will be yes because your wife has 2 passports and as you have mentioned that her initial visa (entry clearance visa) and Tier 1 General visa are on her old passports.

Most recent application was TIER 1 Initial application
Is this Mean POSTAL DATE of my inital application
or
Visa Approval date of inital application
I think HO is asking POST DATE of Inital application not visa stamp or approval date
BECAUSE
Question is most recent APPLICATION not MOST RECENT VISA/STAMP date..... ?
(if HO is asking for Visa Approval date of inital application : HO should asked VISA approval date or visa start date etc )

Dear Members
My wife have Tier 1 general visa and she has recently got her extension .She have 3 dependents and all of us got extension same time.
the 4th dependent is our son who is minor (17 years old) is outside UK.Now she want to apply Tier 1 Dependent visa for him .
My question is how much maintenance ...does she need to show maintenance for for only one dependent who is outside uk ...just note she has been in UK more than one year.
does she need to show only £600 for single dependent??
or she need to show (£900+£600+£600+£600 (herself and 3 dependent already in uk with valid leave)+£600(dependent who intend to apply now)=£3300
please advise me i tried to find out but all the guidance explain if the main applicant and dependent applying at same time.
