Questions of SET(O) Form (Long residence 10 Years)

I am trying to complete the form, and then I reached to the end - section 11, where they asked me to sign to prove my bank/utility bill is 'correct'
My case is relatively simple. I am 9 years student + 1 year PSW.

When I was going through the form, there is no part asking me to provide any bank letter or utility bill. So do I still need to complete this part and provide them some sort of address proof?
