Tier 1 extension application - Queries

I received my Tier 1 visa in July 2009 while I was working here in UK and then left back for India in Oct 2009 to return in Feb 2011.
I am applying for leave to remain under Tier 1 General this month and have a few queries.

Query 1
Page 12 B15-B17 Address details for the past 2 years (24 Months)
I am in UK since past 15 months but before that, I was in India, so shall I put India's Address as well?

Query 2
Page 15 B-27 Occupation
I work as a Test Analyst in IT industry but did not find any suitable occupation from those listed in the form except for Science and Technology.
Is that the right one for me?

Query 3
Page 20 D1 - Did the applicant receive entry clearance/visa before travelling to UK?

So I am confused about my answer for this question. Does it mean JUST before I came here (in that case my answer should be NO) but if not then I think I need to provide my visa details. Am I going wrong?

Page 53 - Maintenance
I hold an account in just one bank. Can I show the same bank statements for both earnings and maintenance OR do I have to get another copy of stamped bank statements for maintenance?

I am applying for Tier 1 ext PEO on May 15.
My case is straight forward .. how long it will take to get receive the biometric card if it is approved on the same day...
