Forms for Tier 1 Extension

HI All,

Me and my wife are going for tier 1 2nd extension next week at PEO solihull, 1)Can any one tell do we have separate form for bio-metric,or its only one form for each of us,

2) How many years visa will i get for 2nd extension, I have 2 years HSMP+ 3 Years Tier1 as of now,

I have see couple of people getting 3 years extension again,

how many years have you completed on HSMP+ TIER-1

I have received HSMP in July 2007, i came to UK in may 2008, i extended my visa in may 2009 and let UK in march 2010 and back to uk in Sept 2011, working from Oct , have 7 pay slips and qualified for extension,

no continuous stay , not eligible for ILR hence going for FLR,
