Employment letter from accountant

I am applying for a schengen visa. I currently work as an IT contractor. I am planning to get an employment letter from my accountant. Could anyone please share with me a sample format of the letter that we need to submit.

Any accountant worth his salt should be able to provide this. Just explain what you need it for.
