Tier 1 visa Extention + 1 dependent

Hello Forum members

I will be applying for my visa extention .
I am working a director in owned company

I draw salary + dividend from the company

Like previous application ill be submitting the following
--------------------------------------------------------------

-Payslips
-Dividend voucher (just wanted to confirm that they still count gross ?)
-Letter from accountant
-Bank statements
-Even though i was working as a director (we considered it being emplyed however during initial application first it was reviewed and case worker stressed i was s/employed so we registered and i have UTR no for the same
-passports
-marriage certificate
-photos




What document do i need from HMRC to show the tax/ni deduction

Also do i need to submit any company accounts ? As my y/e is april/2012 its hard for my accountant to prepare accounts so earlt, although he could provide a draft accounts. our t/o for the year was half a million so its not hard to assess the income i have made. however to be on safe side do we need this
