This is coming from a personal experience. When my wife applied for her dependant visa from india, the VAF office in Ahmeadabad asked for attested copies all 36 pages. Even the blank ones. At first I didn't sent her the blank pages copies but since they refuse to accept the application without all the pages I had to send her the rest of them. So to be on a safer side take copies of all pages.

This looks ok. In my case I also sent 6 months of payslips. Just a thought

Attested copies of all pages are not required. It sounds like you had a bad experience with the vac. You only need to prove the sponsors identity and immigration status not his travel history.
