ILR applied in Mar12. which form for dependent extn? URGENT

Hi,

1> ILR APPLIED for myself at CROYDON PEO - 16-Mar-2012 (Form - SET(O))
2> Biometrics - done
3> Additional document requested from Employer. It has been provided to UKBA on 29-Mar-2012
4> ILR Status - Awaiting

Query:-
My Tier 1 visa is getting expired on Jun 30. I am going to apply for my wife dpendent visa extension. My wife is in UK for the last 1 year.

(i) Which form do I need to use FLR(M) or PBS dependent?
(ii) Do I need to mention that I have applied for ILR & the reference number

Any guidance would be much appreciated, as I need this information very urgently
