SET(M) application - queries

Hello
I will be applying for ILR using SET(M) form - on the basis on married to the British national after finishing 2 years qualifying period in the UK.
I have a few queries on this:-
1. Is this the right form for me?
2. What does BRP Reference number in Section 1 - question 12 stand for?
3. Section 6-question 8 and 9 ask me to give details of when my fingerprints were taken for the biometric Residence Permit. It was taken two years ago but I don't remember the exact dates. Can someone tell me how I can find this out.?
4. I have received a Fixed Penalty on my license with 3 points and a fine of £60 last year. Do I have to give these details? I understand that Fixed Penalty is not a criminal offence. Am I right?
